I just had a request from a book publisher to use one of my designs for a book cover. I now need a contract. Are any contracts available free online? Do I need a Limited Rights Contract, or some other type? Thanks for any and all input.

You could try looking at the AIGA website. It's a graphic design organization, but much of the issues would be similar. Graphic artist's guild also has information and a book on pricing and guidelines which might be worth looking at, especially if you can find it at a library. I think it has some contracts for illustrators. If you can, run any contract by a lawyer. Check for "Lawyers for the arts."
Usually the publisher supplies the contract because they state the terms and you either agree or disagree with them. If you disagree, you negotiate and make changes to their contract before copying and signing it.
